Buick digital climate control part number 16236054 with side mounting tabs and a blue plug on the back.  The ATC you will receive will be cosmetically good but not perfect, if you want perfect buy a new one. If you have a manual heater control now this will not work so please do not purchase it.

The part is from a salvage vehicle and is in all around good condition
