After a dealership technician performed the inspection, he signed & dated the completed form. Typically, it was then filed in the salesman's 'deal folder' for that vehicle. Of course, last year's blanks were eventually thrown away or used as scratch paper. This one came from the files of a closed dealership, where it had been sitting since the model year ended.
